This study aimed to build a patient discharge multiprofessional protocol for people with heart failure and their families. The methodological reference was the Convergent Care Research, which allowed the triangulation between theory, research and practice. The study was developed at the hospital institution, where the researcher worked during nine years. The research was approved by the Research Ethics Committee of the educational institution, CAAE 98300918.0.0000.5142, Process nº. 3.071.413. Data collection occurred in two moments: literature basis, with integrative review and search of data on the websites of hospitals of reference in cardiology, in order to identify the necessary guidelines for hospital discharge of people with heart failure and family members; and the approach to the care practice, through multiprofessional focus groups, whose participants were professionals from the hospital institution, in order to know, from the perspective of each professional, the necessary guidelines for the person with heart failure and families. The patient discharge multiprofessional protocol for people with heart failure and their families was elaborated, through the convergence of the guidelines obtained in the integrative review, websites and focus groups, from the identification of the themes. The protocol was presented to the professionals who participated in the data collection, who evaluated its viability and applicability, according to the institutional reality. The integrative role of the nurse among the other professionals and the person with heart failure and family members and between them and the community is highlighted.
